Registering as a Patient at

Annfield Plain Surgery

If you would like to register as a patient at Annfield Plain Surgery there are 3 steps to doing this.

1. Do I live inside the practice boundary?

Please use the map below as a guide to the Practice Boundary. If you are not within the boundary you will not be able to register with us.

If you are uncertain whether you live within the boundary please contact a member of our Reception Team on 01207 215005 who will be happy to help you.

2. Come in person to Annfield Plain Surgery

You will need to bring the following information with you

​

  • One form of Photo ID with your name and current address

  • 2 recent Bills with your name and address. 

​

Our staff will help you complete the forms required for registration.

​

If you would like to save some time doing this you can download the Patient Health Questionnaire below which you can print off and complete before you attend.

3.  Arrange a new Patient Medical with the Practice Nurse

Most people think that there notes from their previous practice arrive as soon as they register. Nothing could be further from the truth!

 

It often takes up to 6 weeks or longer to get hold of your records during which time we know almost nothing about you.

​

An appointment with the Practice Nurse helps us get some very useful information about your health which will be a great help to you while we wait for your old notes to arrive.

​

For this reason you will not be formally registered with us until you have attended this Medical and hope you understand why this is so important for you and your onpoing Healthcare.
